- Referrals to residential care need to come from a CDHB Specialist Mental Health Services Case Manager who is prepared to stay involved for the duration of the client’s involvement with Stepping Stone. All services require a phone call followed by a written referral prior to admission.
- Residential referrals also require that a recent psychiatric assessment and crisis plan be provided as part of the referral information. This assists us in delivering the very best care to young people who come to stay with us.
- There is currently no requirement that a person have a completed Needs Assessment. This is facilitated as part of co-ordinated meetings between referring agencies and service providers on a weekly basis.
- Respite requires a range of other forms be completed by a person.
